If you are a current resident of Collier County, you may complete a Florida Voter Registration application form. Click HERE to complete an application designed for printing and mailing. This form can be used to submit a new registration or to change your existing registration information (i.e., party affiliation, address changes, name changes, etc.).
When you have completed the form, you must print it, sign it and return it to the Supervisor of Elections office of Collier County, located at:
